On a new mold, it is more likely that manufacturing has caused an error making the cores.

It is imperative to ensure water cooling in the cores is flowing properly, and at a sufficient pressure and flow. Also always ensure that no gate nubs are present during mold closing, or applying tonnage could bend the cores. Over packing of the preform could also deflect the core rods, so ensure your internal orientation looks proper. Acceptable amount of deflection depends on your wall thickness. I would certainly not exceed 10% of the wall thickness.
